SECTION 1: RX PLAN INTRODUCTION The Plan Sponsor has adopted and established this CODAC Health, Recovery & Wellness, Inc. Prescription Drug Benefit Plan (which we will refer to as the "Rx Plan" or the "Prescription Drug Plan") for the purpose of providing prescription drug and pharmaceutical health and welfare benefits to eligible Employees under the terms stated herein. This document, along with its Exhibits establish the Rx Plan for the Plan Sponsor. This Rx Plan is intended to satisfy the Employee Retirement Income Security Act of 1974 (ERISA) requirements for both a Plan Document under ERISA Section 402 and a Summary Plan Description ("SPD"), pursuant to ERISA Section 102.
