What are some examples of expenses not eligible with an FSA? • Expenditures that are merely beneficial to the general health of the person • Amounts compensated for by insurance, government agency, or workers’ compensation • Cosmetic surgery, other than that needed to improve congenital abnormality, personal injury, or disfiguring disease What happens if I don’t use all the money I set aside each plan year? Depending on your plan, you may be able to carry over part of your remaining balance or use funds for a limited time the following year — see your Summary Plan Description for details. Unused funds may be forfeited, so keep that in mind when making your election and check your balance regularly at myplans.cbiz.com or on the My Plans by CBIZ app. Once I make an election, can I change the amount during the plan year? No, you can only change your election during the year if you have a qualifying status change, such as marriage, divorce, a change in dependents, or a change in employment, and the change must be consistent with your new election. Do I need to submit receipts with my reimbursement request? Yes, you need to submit a provider statement showing the service date (not payment date), type of service, provider’s name, and amount claimed. An explanation of benefits from your insurance is also acceptable. Canceled checks or credit/debit card receipts are not valid proof. Can I use my account to pay for uncovered health and dental expenses incurred last year? No, only expenses incurred during the plan year can be paid for with your account. Expenses are incurred at the time of service, not the time of payment. What expenses can I pay with my account? Any approved medical expenses that are not reimbursed under your insurance coverage. Can I also deduct these medical expenses on my income tax return? No, you cannot deduct these expenses on your income tax return and also pay for them with your FSA.

Does participating in this plan affect the filing of my income tax return at year end? No, your tax savings are adjusted on your Form W-2 and there is no extra work filing your income tax returns. Will I need to repay these taxes in the future? No, you never have to repay the taxes you save with this program. If my insurance covered half the bill, can I withdraw the full amount of the bill from my account? No, you cannot withdraw funds from your FSA for expenses already covered by insurance, but you can use it for the remaining balance after insurance. Why would I want to pay these expenses under the company’s FSA plan? Paying eligible expenses with an FSA lets you save on taxes and increases your available take-home income. If I’m not covered under the company’s health insurance plan, may I still participate in the FSA plan? Yes, your health insurance provider doesn’t matter — you can use the plan to pay for any medical expenses not covered by group insurance for any family member.. How do I receive the pre-tax money that has been withheld from my paycheck? You can file a claim online at myplans.cbiz.com or through the My Plans by CBIZ mobile app. How do I keep track of my contributions and the amounts I have been reimbursed during the year? You can view plan information anytime at myplans.cbiz.com. What happens if I quit or am terminated from the company? You may be able to continue to submit claims up to your termination date for reimbursement. Will my spouse have access to my account?
