Please contact the benefit manager or Your employer with any questions related to this coverage or service. The Plan covers eligible services (“the Services”) as part of the Plan’s Envita Medical Center’s (Envita) Cancer 2nd Opinion and Cancer Treatment Program. This program may provide access to Envita for certain cancer conditions.

The 2nd Opinion is provided at no cost to Covered Persons; however, if after the 2nd opinion the Covered Person commences cancer treatment at Envita all treatments and services will be subject to the Plan’s payment provisions and coverage rules. Prior authorization requirements and some exclusions are waived when receiving care at Envita. Participation in this program is voluntary and is subject to the Covered Person meeting Plan eligibility requirements. In order to participate in the program, the Covered Person (or the Covered Person’s parent or legal guardian) must:
agree to abide by program requirements

• be able to safely travel for medical care and not require Emergency care at the time of travel • identify and provide a designated caregiver(s) who can support the Plan Participant
Overnight lodging at a local Envita negotiated hotel, when the patient travels more than 50 miles (one way) to receive treatment at Envita, is covered by the Plan. Combined overnight lodging costs incurred throughout the completion of the Individual Patient Protocol will not exceed $10,000 under this agreement. Combined lodging costs greater than $10,000 as well as incidental costs associated with lodging such as food, beverages, entertainment, and parking are not covered by the Plan and are the responsibility of the patient.
Not all cancer conditions, including pediatric cancers, are eligible for this program.
