• Investigative Suspension – Utilized only with legal counsel guidance. • Termination – Termination is the last and most serious step in the disciplinary action process. Generally, CODAC will try to provide coaching opportunities, warnings, performance improvement opportunities prior to proceeding to a recommendation to terminate employment. However, CODAC reserves the right to combine and skip steps depending on the circumstances of each situation and the nature of the offense. Furthermore, employees may be terminated without prior notice or disciplinary action. Employee terminations must be reviewed by and coordinated with Employee Relations in advance. Policy Reference: • HR-Perfmgmt-554 – Disciplinary Actions and Procedures WORK-RELATED INJURIES OR ILLNESSES An employee who sustains a work-related injury or illness should inform his or her supervisor immediately. No matter how minor an on-the-job injury may appear, it is important that it be reported immediately. Employees who sustain work-related injuries may receive workers' compensation benefits outlined in CODAC's Workers' Compensation policy (HR-Benefits-554). Employees who need to take time off from work due to a workers' compensation illness or injury may also be eligible for a leave of absence under CODAC's leaves of absence or reasonable accommodation policies. Employees should contact Human Resources for additional information. When work-related accidents, injuries or illnesses occur, employees may be eligible for workers' compensation insurance benefits. CODAC provides a comprehensive workers' compensation insurance program at no cost to employees and in accordance with applicable state law. This program covers most injuries or illnesses, sustained in the course of employment, that require medical, surgical or hospital treatment. Subject to applicable legal requirements, workers' compensation insurance provides benefits or, if the employee is hospitalized, treatment immediately. REPORTING WORK-RELATED INJURY OR ILLNESS Employees who sustain a work-related injury or illness should inform their supervisor immediately. No matter how minor an on-the-job injury may appear, it is important that it be reported immediately. This will enable an eligible employee to qualify for coverage. WHAT TO DO IN THE EVENT OF A WORK-RELATED INJURY If an employee has sustained an injury or illness on the job, you must immediately report the immediately – even if the employee did not seek/want medical attention. 1. Contact the Human Resources department immediately during work hours but no later than24 hours following the accident or injury or within being notified of accident or injury. 2. After-hours contact Chief Human Resources Officer (520.403.2790) immediately. 3. “Any” work-related accident and or injury requires a post-accident drug and alcohol test whether it is a vehicle-related or non-vehicle-related. Human Resources will arrange for the referral. 4. Employee and Supervisor must complete the Accident/Incident Reporting Form (Non-Vehicle Accident) or Vehicle Accident Employee Report (Accident and/or injury) and Witness Statement, if applicable.
