CBIZ Flexible Benefits Debit Card The CBIZ debit card provides direct access to your Flexible Spending Account (FSA) funds, deducting purchases for goods and services from your available balance to pay providers and minimize out-of-pocket costs.

How to Manage the CBIZ Debit Card

» There are now over 40,000 IIAS-certified retail locations; a full list is available at www.sig-is.org. » Purchases made at IIAS locations usually do not require a receipt for verification.

• The CBIZ debit card is only accepted at qualified locations where Visa debit cards are accepted. » Qualified locations include hospitals, doctor’s offices, pharmacies, online drug stores, and day care centers. » Card swipes are processed as credit card transactions. PINs may be requested when the card is received. • The CBIZ debit card is not a fully paperless process. The IRS requires receipts for most transactions to verify expense eligibility due to FSA plan tax benefits. » Certain transactions may be automatically verified without requiring documentation if they match the plan’s copay amount or are made at an IRS-certified retailer or pharmacy, also known as an Inventory Information Approval System (IIAS) merchant. » For other transactions, CBIZ will notify you to submit documentation to verify the expense. » All receipts must include the provider or merchant name, original date of service, type of service or purchase, and amount charged. Simple debit card receipts are not acceptable under IRS guidelines. • The IRS requires drug stores and pharmacies to be IIAS certified, meaning able to identify eligible healthcare products at checkout, unless they meet the 90% rule.

• Participants receive an email or text notification after a card transaction is processed if a receipt is required.

» Purchases needing substantiation can be viewed by logging into myplans.cbiz.com. » Users can review all submissions and upload required receipts directly in the portal.

• CBIZ sends monthly reminders until adequate documentation is provided for a charge.

» If CBIZ hasn’t received a receipt within three months, the debit card will be suspended for that plan. » Charges older than 120 days without documentation will be marked as non-qualified expenses and must be repaid to the flex account or will be deducted from future reimbursements.

• Once the issue is resolved, either through documentation or repayment, the card will be reactivated.
