education and training, communication, and enforcement of policies and procedures for infection control and prevention. • Business Continuity Plan - establishes procedures to recover the business following a disruption such as utility failure, disaster or unexpected site closure. The business continuity plan is critical in keeping CODAC open to provide care for the community when it is most needed following an emergency or other service interruption. • Accessibility Plan – provides a means to facilitate continual quality improvement around accessibility. CODAC is committed to providing an organizational milieu that seeks to accommodate the needs of all persons served, employees, and stakeholders. Central to this commitment is the removal of architectural, attitudinal, employment, technological and other barriers that may impede full access to the services and programs of the organization. • Strategic Plan – Establishes the high-level areas of focus that the organization has prioritized based on community, staff and member needs; environmental threats and opportunities; and agency strengths. The Strategic Plan outlines the environmental issue or opportunity being addressed, the overarching strategy CODAC has identified to address the environmental issue or opportunity; measurable and time specific target outcomes and priority level. Updates are made on a quarterly basis. • Risk Management Plan - assesses, manages and identifies means to decrease potential and actual risks to persons served, employees, and visitors of CODAC • Technology and Systems Plan - CODAC’s existing technology infrastructure and usage in order to make recommendations for future planning and direction. • Quality Measurement and Management Plan – ensures that CODAC is demonstrating the value of care management programs, both to ensure that services provide value to persons served and other stakeholders. This includes improved health outcomes for members, efficient use of services, provider adherence to evidence-based standards of care, access to care and slowed spending growth. • Performance Improvement Plan – ensures CODAC encourages a system that concentrates on performance improvement initiatives around critical areas, including clinical outcomes, member experiences, staff experiences, access to care and organizational costs. • Workforce Development Plan – ensures that CODAC establishes a process and opportunity to improve health outcomes by enhancing the training, skills, and performance of staff. MEETINGS & COMMITTEES CODAC has a variety of committees dedicated to ongoing performance improvement. Some of these meetings or committees may require supervisor approval prior to attending. Operation Efficiency Committee (OE) The Operation Efficiency (OE) committee is co-chaired by the Chief Compliance Officer and the Chief Clinical Officer as a backup. The committee meets weekly and is attended by all chairs and co-chairs of CODAC’s Quality Improvement Program sub-committees. Attendees include QM and UM teams, the President/CEO, the Chief of Information Technology, Chief Human Resources Officer, the Chief Clinical Officer, and designated clinical directors. This committee address a variety of things impacting CODAC as a whole, but with regard to UM, the following are the goals of the QM/UM involvement in this meeting:
