1) Present analyses of utilization data for discussions by the departments represented at the meeting; 2) Ensure an appropriate, responsive, effective, and efficient service delivery system; 3) Identify problem areas and discuss action steps to resolve system problems; 4) Identify areas which appear appropriate for study of utilization trends and patterns related to under- and over-utilization; 5) Discuss any issues regarding network and contract compliance and discussion of outcomes; 6) Proactively review complaint, grievance and appeal data to identify outlier members with multiple complaints, grievances or appeals regarding services or against CODAC or who contact governmental entities for assistance, including ADHS and AHCCCS; 7) Review medication errors and recommend corrective actions and monitor and report on the effectiveness of actions to resolve individual member, provider, and system problems. Meeting Frequency: every other week or at minimum once a month; or as needed/closed meeting. Information Systems and Technology Committee (Data Security) The Information Systems and Technology (IS&T) Committee is a subcommittee of OE. The Chief Information Officer and Director of Informatics co-chair this committee and the role and function of this committee is to: 1) identify and assess risks in the information systems and information technology area of the organization such as network security, technology and hardware becoming outdated, and hardware that may need replacement; 2) plan and oversee the mitigation of the IS&T risks identified; 3) conduct an independent network and server audit every 24 months to identify issues, prioritize the findings, and plan mitigation to correct these findings; 4) participate in the budget planning for the next year’s IS&T capital budget. Meeting Frequency: meets at least annually/closed meeting. Sentinel Events Review Committee (SERC) The Sentinel Events Review Committee (SERC) is a subcommittee of OE. The role and functions of the committee are to: 1) review and investigate any patterns or trends identified in the review of sentinel events; 2) assess the need for corrective action plans; and 3) plan, implement, and document any plans of correction that would enhance the safe practice and provision of agency services. CODAC uses SERC findings to target improvement activities. CODAC also receives feedback about Incidents/Accidents/Deaths from various stakeholders (e.g., health plans, CARF, AHCCCS, NAMISA, Arizona Center for Disability Law, DDD, etc.). Meeting Frequency: meets at least bi-annually/or with supervisor approval.
